Looks to be a multiple exposure. This is the kind of stuff camera students used to produce back in the 1960's. Notice there's even a note dated march of 1964. Most likely an art project.

The note also has the year 1999 on it. I think it is more than a 60's art project.

If you put this picture into a photo editor like Paint shop, you can see more detail by changing contrast and brightness. I was able to make out James, Michelle. I still wasn't able to find the Artist.

There's also definitely an image of an MRI or CAT scan of a person's head/brain in there, too. Maybe it's an artistic representation of a person struggling with some kind of medical problem, such as schizophrenia?
